To request your hospital grade breast pump, please fill out the form below. Our office will contact you to go over our rental contract and process the first month’s payment. We cannot ship a pump until we have received a completed rental contract and financial policy, and a full month's rental is paid upfront. The rental contract and our financial policy will be forwarded to you via a Formstack secure email to obtain your information and signature. We will bill you monthly after that using the information you provide on our rental contract.
