Steps to order your compression garments through insurance:

  1. Fill out the form below to let our office know you are interested in compression garments through your insurance.

  2. Have your doctor fax us this prescription for your compression garments to 401-884-5541.

  3. Once we have both an order form and a prescription, our office will call to schedule an appointment in our East Greenwich, RI office. During your appointment, one of our nurses will ensure you are properly measured and fitted and provide you with the compression garments to take with you.
